This event is jointly provided by the Chronic Care and Disease Management Program at the Rhode Island (RI) Department of Health and the Office of Continuing Medical Education at the Warren Alpert Medical School.
Many provider practices have limited understanding of why quality improvement (QI) practices are not always successful nor are they always able to narrow down the problems in work flow to incorporate QI. Lauren Capizzo, MBA, CPEHR, PCMH CCE and Brenda Jenkins, RN, D.Ay, CDOE, CPEHR, PCMH CCE, from the New England QIN-QIO, will present an overview on this, and how practices can identify the most appropriate problems to incorporate QI and tools to best measure success.
